AnchorPoint Transitional Systems™ equips youth preparing to leave care with a structured, measurable roadmap for independent living—so they don’t just transition, they launch.

✔ Youth preparing to age out within 6–12 months
✔ Organizations seeking structured transition outcomes
✔ Programs wanting measurable readiness—not just participation
Structured Stability. Measurable Independence.
Through years of service within child welfare systems, a consistent pattern has emerged—youth preparing to age out of care often leave with information, yet without a structured, actionable plan for independent living.
AnchorPoint Transitional Systems™ was developed to address that gap. This is not a life skills class. It is a structured, measurable transition model designed to ensure youth leave care with a documented plan for stability, accountability, and long-term success.
The Life Launch Blueprint™ is a 6-month structured program designed for youth ages 16–21 preparing to transition out of care.
Each participant is guided through a defined process that results in a personalized, actionable plan addressing:
• Housing readiness
• Employment preparation
• Financial organization
• Emotional regulation
• Support network development
Youth do not simply receive information—they leave with a documented roadmap for independence.
• Live, virtual group sessions (twice per month)
• Structured guidance through core life domains
• Ongoing accountability and goal tracking
• Measurable readiness scoring through the AnchorPoint Readiness Index™ (ARI)
• Development of a personalized Life Launch Portfolio™
Each component is designed to move youth from exposure to execution.

The AnchorPoint Readiness Index™ is a structured scoring framework used to measure each participant’s progress across five key areas:
• Financial Stability
• Employment Preparedness
• Housing Readiness
• Emotional Regulation
• Support Network Strength

AnchorPoint is built around the reality of child welfare systems. Enrollment is tied to the youth—not the placement. If a youth’s placement changes during the program, they are able to continue their progress without disruption, ensuring continuity of support and completion of their Life Launch Blueprint™.
AnchorPoint is designed for:
• Youth ages 16–21 preparing to age out of care
• Independent living programs
• Group homes and residential providers
• Child welfare organizations seeking structured transition support
AnchorPoint was developed through years of direct work within child welfare systems, identifying consistent gaps in how youth are prepared for independent living.
This is not a conceptual model—it is a structured response to real, observed needs.
